PLEASE READ CAREFULLY Memorandum of Understanding & Course Contract Prerequisites ● Must be at least 18 years of age or turn 18 within 3 months of finishing the course. ● Must have a high school diploma or equivalent. Job Placement ● KLR Medical Certification Training School does not offer direct job placement. Drop Policy ● NO refund is given after payment is made. Attendance ● Attendance for all classes is required. Unless authorized by Instructor. ID ●Student must have a valid ID in order to take this course. ●This cannot be a paper copy and must not be expired. Background Check ●Students must go through a background check during the licensing process ●If a student has a criminal background they MUST inform administration prior to registration. Licensing ●KLR only operates in the following states: ●Indiana, Missouri, Tennessee, Oregon, Kentucky, Texas, DC, North Carolina, Michigan, Massachusetts, South Carolina, Arizona ●Licensing is only available in these states if a student moves during or after a course it is their responsibility to obtain licensing on their own. *Student will be asked to sign the official contract upon registering for the course* This course does not offer transferrable credits or provide dual credit with any colleges/universities.
